NOTES : Curtis was the son of James Hayes & Hannah Rigby, of Palmerin Street, Warwick. He attended Warwick State High School and was working for the railway at the time of his enlistment in August 1915.

He undertook seven months’ training in Egypt, before departing for France in the October of 1916. Originally Curtis was a member of the 11th Light Horse, 4th Brigade, but was later transferred to the 15th Battalion while still in Egypt.
